미백 약제의 농도와 효능에 대한 입장
 

미(美)에 대한 관심이 높아지면서 미백 치료는 매우 일반적인 치과치료의 하나가 되었다. 미백 치료는 1800년대부터 시작되어 현대적인 미백 치료 방법이 1980년대 후반에 소개된 이후 매우 안전하고 보존적이며 효과적인 술식의 하나로 인식되어 왔다.
 
현재 일반적으로 시행되는 미백법은 1) 치과에 내원하여 시행하는 치과내 미백(in-office bleaching) 2) 치과에 내원하여 제작한 트레이를 이용하여 치과의사의 감독하에 환자가 집에서 시행하는 자가 미백(home bleaching), 그리고 3) 전문가의 감독 없이 환자 스스로 약제를 시중에서 구입하여 시행하는 일반 자가 미백(over-the-counter bleaching)이 있으며 여기에 사용하는 약제의 종류와 도포 방법, 농도는 매우 다양하다.
 
흔히 치과에 내원하여 시행하는 치과내 미백의 경우 해외에서는 35~50%의 과산화수소나 35~40%의 카바마이드 퍼옥사이드를 이용하며, 집에서 시행하는 자가 미백의 경우 5~22%의 카바마이드 퍼옥사이드나 6% 이하의 과산화수소를 이용한다. 일반 자가 미백의 경우 이보다 낮은 3% 내외의 과산화수소나 10% 정도의 카바마이드 퍼옥사이드가 많이 이용되고 있으나 이는 각 나라의 법적 규정에 따라 차이를 보이고 있다.
 
미백시 고농도의 약제를 사용하거나, 열이나 빛을 가하는 방법 등은 빠른 시간 내에 치아가 밝아지는 효과를 기대할 수 있으나 최종 결과에 있어서는 저농도의 약제를 장기간 사용하는 것과 유사한 결과를 보인다고 알려져 있다. 고농도 제품이나 부가적인 광원을 사용할 경우 술 후 과민증, 점막의 손상, 술 후 일시적 통증, 치수 온도 상승 등의 부작용이 나타날 수 있으므로 주의가 필요하다. 따라서 미백 방법을 선택할 때 환자의 치료 가능 기간 및 협조도, 치아의 과민반응 여부, 수복물의 상태, 치주 상태 등을 복합적으로 고려하여 치과의사와 상의하여 최선의 방법을 선택하는 것이 매우 중요하다.
 
가장 흔히 나타나는 부작용 중 하는 치아의 과민 반응인데 이에 대한 정확한 기전이 확립되지는 않았지만 현재까지의 연구에 의하면 퍼옥사이드 성분이 치아의 미세한 틈을 통해 치수에 도달하게 되면 치수에 일시적인 염증을 유발하여 그 결과 치수가 민감해지는 것으로 알려져 있다. 또 다른 흔한 부작용의 하나는 점막 자극 손상이다. 원인으로는 미백 약제의 장시간 접촉 혹은 잘 맞지 않는 트레이를 오랬동안 장착한 경우 흔히 나타나는 것으로 알려져 있으며 특히 적절한 치은 보호 없이 임의로 고농도의 미백제를 이용하는 경우 치은 점막에 화학적 화상이 발생하는 것으로 알려져 있다. 그러나 과민반응이나 치은 점막의 손상은 대부분 일시적인 것으로 영구적인 손상을 유발하지는 않는 것으로 보고되고 있다.
 
안전성에 관한 연구에 따르면 과산화수소에서 방출되는 자유라디칼은 세포 독성(toxicity)이 있는 것으로 알려져 있다. 이에 대해 돌연변이 유발(mutagenicity), DNA 손상으로 인한 유전자 독성(DNA damage, genotoxicity), 발암물질(carcinogenicity)등의 가능성이 연구되어 졌으며 일부에서는 그 독성이 인정되고 있으나 치과에서 사용되고 있는 약제의 양은 이러한 문제를 유발하는 것보다 훨씬 적은것으로 알려져 있고, 지난 20여년간의 임상 사용에서 아직까지 이러한 부작용이 보고된 바가 없어 치과의사의 지도하에 사용한다면 안전성 문제는 거의 없다고 볼 수 있다. 구강 내에 적용된 약제가 치아 외에 노출되는 경우 타액내의 효소에 의해 중화가 일어나고, 만약 환자가 약제를 삼키는 경우에도 그 양이 미미하여 실제 실험실에서와 같이 독성이나 발암물질로 작용한다는 보고는 없다.
 
또 다른 부작용으로 약제 적용 후 치아의 법랑질 표면의 변화가 보고되고 있으나 이는 미백제의 농도에 따른 영향보다 산도(pH)가 더 큰 영향을 미치는 것으로 알려져 있어 안전한 제품의 선택이 중요하다 할 수 있다. 그러나 일반적으로 미백과정 중에 변화된 법랑질 표면은 인체 내에서는 타액의 완충능력과 재광화 과정을 통해 회복되므로 실제 치아에 손상을 준다는 임상적인 보고는 없다. 수복물이 있는 치아에서 미백을 시행하는 경우 글라스아이오노머나 콤포머 혹은 레진 수복의 경우 재료의 물성이나 접착 과정에 영향을 받는 것으로 알려져 있으며 도재 수복물 표면도 영향을 받을 수 있다고 알려져 있으며 기존의 수복물 상태가 불량하여 미세누출이 있거나 우식이 진행되어 있는 경우, 혹은 치근의 상아질이 노출된 경우에는 미백약제의 치수 영향이 커질 수 있으므로 치과의사와 상의가 반드시 필요하다.
 
이처럼 미백은 치과의 치료 술식 중 가장 안전하고 보존적인 술식의 하나로 여겨지고 있으며 2015년 현재 식약청에 등록된 제품은 의약품으로 17종, 의약외품으로 44종이 등록되어 있다. 그러나 치료 방법과 기간, 약제의 농도, 구강내 상태 등에 따라 부작용이 나타날 수 있으며 환자가 치아 변색의 원인에 대한 정확한 진단 없이 임의로 장기간 시행할 경우 원하는 결과를 얻지 못하고 오히려 질환을 더 악화시킬 수 있으므로 사전에 치과에 내원하여 검사 후 시행하는 것이 바람직하다.
 
이러한 현실을 반영해 볼 때 대한치과보존학회는 미백 치료를 시행하는 전문가의 입장에서 미백약제의 농도에 따른 효능과 안전성에 대하여 과학적인 논리에 근거하여 다음 입장을 밝히는 바이다.
 

  1. 1. 모든 미백 술식의 시행은 시행 전에 원인에 대한 정확한 진단이 우선 되어져야 하며 이는 전문적인 지식을 가진 치과의사의 검사를 통해 이루어진 후 시행되어져야 한다.
  2. 2. 제품 농도와 미백 효과에 대하여는 논란의 여지가 있으므로 방법의 선택은 전문가와 상의하여 결정하는 것을 추천한다.
  3. 3. 일반 자가미백용 저농도 제품이라도 전문가의 지도 없이 장기간 사용하는 것은 피해야 한다.
  4. 4. 임산부, 모유수유 중인 여성의 경우 미백을 하는 것을 출산 후나 모유수유가 끝난 이후로 미룰 것을 권한다.
  5. 5. 소아-청소년의 경우 혼합치열기 아동은 영구치열이 형성된 후로 미백을 미루는 것을 추천하며 영구치열이 완성 된 이후에는 치과의사 감독하에 미백을 시행할 수 있다.

다음의 환자는 미백을 시행하는 것을 피해야 한다.
  

  1. 1. 퍼옥사이드 계열에 알러지가 있는 환자
  2. 2. 임산부, 모유수유중인 환자
  3. 3. 구강건조증 환자
  4. 4. 치근이 많이 노출되어 있고 치아 과민증을 보이는 환자
  5. 5. 충치로 인해 변색이 있는 환자(충치 치료 시행 후 미백 가능)
  6. 6. 중등도 이상의 치주질환이 있는 환자(치주 질환 치료 후 미백 가능)
  7. 7. 천식이나 다른 호흡기 질환이 있는 환자
  8. 8. 조절되지 않는 당뇨나 암이 있는 환자
